Health plans now request a chart pull for more than 60% of Medicare Advantage members each year to document diagnoses needed for risk adjustment. That process is cumbersome, costly and a source of provider abrasion. Given the growing volumes of clinical data that plans receive, it is also increasingly unnecessary. In fact, what plans receive today in response to a chart request is commonly an electronic document — containing clinical data that already resides elsewhere in one of their systems.
